How Does Senior Support Solutions Work With You?
Assessment
Initially, we will meet with the older adult client and/or those involved, to complete a comprehensive assessment. At this meeting we have an opportunity to meet, and get to know one another as we gather pertinent information. We will review the physical, functional, social/emotional and medical needs of our client. Areas of particular concern to each of you will be discussed. Identification of supportive care needs will be reviewed and the current living arrangement will be evaluated.
Care Planning
Upon completion of the assessment, a comprehensive care plan report will be created. This written report will outline recommendations for a safe plan of care. It will take into careful consideration our senior client's history, personality and finances. We at Senior Support Solutions will provide you with a list of care recommendations, as well as specific services and facilities that will best meet your needs. At times it may be appropriate for us to gather additional information from medical, legal or financial professionals familiar with the older person's circumstances. These contacts will be made only after the responsible party has signed a Release of Information form.
Implementation
The next step will depend on your preference. We are happy to present you with our written care plan, which you can implement yourself or, if you would prefer, we can implement it for you. We can arrange all services and see that every detail is attended to. Typically, we suggest a planning meeting to review the care plan and discuss how you would like to proceed. At that point, we will provide you with a contract, which will delineate our service agreement.
Monitoring
In addition to putting the services in place, we can also provide on-going monitoring. Senior Support Solutions care managers provide care coordination, scheduling, supervision and advocacy for our clients. We can be your eyes and ears and keep you informed on a regular basis. We can stay involved as long as you would like.
Ongoing Evaluation and Reassessment
As we stay involved with the monitoring of our senior client, we will periodically reassess the care needs, and when appropriate, recommend adjustments to the care plan. In order for you to feel comfortable with the care you or your loved one is receiving, it will be important for us to remain in regular contact. As part of the contracted service plan, we are available by telephone, faxes, E-mail, and mail.
